Yes, we received our License by the State of Georgia (The Department of Community Health).in 2016. And, we cover
twenty-three counties in Metro Atlanta.
We provide personal care to the client in their home environment: Our specific tasks are, ADL care such as, assisting with
bathing, dressing. reading, meal prep, light housekeeping, laundry, medication reminder, trips to MD
appointments,errands, shopping, and monitoring baths and clients in the home for safety, etc.

We can get you a PCA/Sitter by tomorrow, our Nursing Director will accompany the Aide on the first day to develop and
implement a Service Plan geared to your needs.
Supervisory visits are done every 62 days for Personal Care Services. And,122 days for Companion and Sitter Services
Yes, we receive verbal or written Physician Orders. Then, within 24 hours the Nurse Director will verify the orders, give
you or your family member a call,and put in place a plan of care as per your needs
Please give us a call and we will promptly address the lateness. “No show”? You are given the option to have us send
another caregiver, or you may accept credit for the absent care giver.
We accept Medicaid under the Medicaid Waiver Program called CCSP & SOURCE. Clients that don’t have Medicaid will
have to pay-out-pocket by the family. We will then assist the family in applying for Medicaid.
Our fees are affordable, we will sit down with the client and their family members to work out a plan that’s affordable. We
have easy pay via Visa, Zelle, or,simply mail us a check for services rendered.

SNPHCS pays all our staff members for their home care services provided. Reimbursement for transportation trips are
added into your fees, therefore, there are no extra outer pocket expenses to you.
